TO: Jessica Dawn Munn
Take notice that a pleading seeking relief against you has been filed in the above entitled actions. The nature of the relief being sought is Termination of Parental Rights.
Pursuant to N.C.G.S. Ann. § 7B-1106: (a) counsel already representing you in an abuse, neglect or dependency proceeding will continue to represent you; (b) that if the you are indigent and not represented by appointed counsel the you are entitled to court appointed counsel; (c) that if provisional counsel has been appointed the appointment of provisional counsel will be reviewed by the court at the first hearing after service; (d) and that notice of the date, time and place of any pre-trial hearing pursuant to N.C.G.S. Ann. § 7B-1108.1 and the hearing on the petition will be mailed by the petitioner upon the filing of the answer or 30 days from the date of service if no answer is filed. You have the right to attend the termination of parental rights hearing.
You are required to make defense to such pleading not later than the __ day of ____ , 2016, said date being 40 days from the first publication of this notice; and upon your failure to do so the party seeking service against you will apply to the court for the termination of your parental rights.
